Here at BPOC, we welcome all patients with or without appointments. Walk-in patients, please be advised that your wait time may fluctuate according to the flow of previously scheduled appointments. Your appointment length will vary depending on treatment needs and scope of services being provided to you that day.

If you cannot make your appointment or have missed a scheduled appointment, please call us as soon as possible to reschedule.

NOTE: All new patients should arrive 15 minutes before your appointment time to keep appointments timely.

BEFORE YOUR VISIT

PRESCRIPTION/REFERRAL REQUIRED

In order to receive any services, you MUST have a prescription and/or approved authorization at your first appointment at our facilities.

Please be aware, some insurances may take between 10-15 business days to process an authorization for any orthotic or prosthetic devices.

If you are a new patient or if it has been over a year since your last appointment with BPOC, pre-registration is required. You must bring the following items to receive services/products, without any of these items, we will not be able to help you.

  • Current insurance cards and State ID

  • Physician’s prescription and/or approved Authorization

  • For Worker’s Compensation cases, bring case manager contact information, workers comp number and DOI (date of injury).

  • A valid form of payment - cash, check, or a major credit card (VISA, Mastercard, AMEX, & Discover accepted). A 3% transaction fee applies to all debit and credit card transactions, no exceptions.

  • Patient Registration Form - available for print by clicking HERE and can be brought in at the time of your appointment.
